Returns… Need to return an item for credit or exchange? • Call our Customer Service Department within 30 days of the invoice date for a RMA number. • Please note, returns past 30 days from invoice date are subject to a restocking charge. • Only items listed on the RMA form will be processed for credit. If additional items are added, a restocking fee will be applied. • We will provide a return label and return instructions via email, fax or postal mail. • Include a copy of the original invoice and the reason for the return with the item being returned. • Customer is responsible for all freight and shipping charges with returns. • Items damaged or lost in transit require claims to be filed directly with the carrier. • Items must be returned in unmarked, unopened, and original sealed packages. Exceptions… The following items are NOT eligible for return under any circumstance: • Opened handpieces, small equipment, large equipment, associated accessories, and custom-ordered equipment. NOTE: Before opening a sealed item, please examine the label to determine you have received the correct item ordered. These items can only be replaced according to the manufacturer’s warranty as directed by federal regulations. • Special ordered items (non-stocked) for products DHPI does not ordinarily stock. • Personalized, customized, imprinted items or government-regulated items. • Opened computer hardware and software. • Hazardous items. • Short dated or expired product. • Discontinued merchandise. • Items marked as non-returnable. • Items that cannot be returned to the manufacturer. • For x-ray film you believe to be defective, please call the manufacturer’s help line direct. Defective x-ray film cannot be returned to DHPI. • Certain Health-Tec ® items such as bulk items, alloy, or imprinted product. Please see our Health-Tec ® Guarantee for more information. No claim for shortages will be allowed after 48 hours of receipt of delivery. We will only grant full credit on items we can return to the manufacturer for full credit or those that are returned in resalable condition. Re-stocking fees and other restrictions may apply. We reserve the right to refuse any unauthorized returns following inspection. Please call our Customer Ser- vice Department for more details, 800-626-2163. Health-Tec ® Guarantee… We stand behind our Health-Tec ® Brand Products by offering you a 100% satisfaction guarantee. If you have tried a Health-Tec ® Brand Product and it is defective or doesn’t perform to your expectations we will provide a credit, refund or exchange. Please call our Customer Service Department to explain the reason for the return. We have a few simple conditions that apply: • Product(s) can only be returned within 60 days of the invoice date. • Discontinued or personalized merchandise.
• Hazardous, personalized, or special ordered items are not return- able under any circumstance. • If you choose to return the item, DHPI will assume responsibility for any common-carrier ground shipping or restocking changes that may apply. • Your account will be credited, a refund issued, or an exchange of goods will be made once the return has been received, inspected and approved by DHPI. Regulatory Requirements… Local regulatory requirements may apply to use or installation of certain products. Be sure to understand and comply with any such requirements prior to purchase, use, or installation of products. Any “transfers of value” made to you by DHPI or other distributors or manufacturers of dental care products may need to be reported to the Center for Medicare & Medicaid Services (CMS).
