THE ROLE SMART PURCHASING CAN PLAY IN DENTAL OFFICE ECONOMICS
Working to control practice expenses is a high priority for any dentist. Overhead can be reduced by increasing productivity, revenue and reducing expenses. According to the American Dental Assistants Association, the DA is primarily responsible for maintaining and ordering dental supplies. The DA makes the sourcing decision in 60% of the cases and influences the choice of supplier in 35% of the more ‘technique sensitive’ supplies. The average dental office generates between $700,000 to $1,000,000 in revenue each year and dental supplies represent 7% of all operating expenses. In this example, dental supplies represent $49,000 to $70,000 of office operating expenses. What would your dentist think if you could reduce his/her supply expenses by 25-35%? The duties of a dental assistant are among the most comprehensive and varied in the dental office. The dental assistant performs many tasks requiring both interpersonal and technical skills. Although state regulations vary, responsibilities may include:
• Assisting the dentist during a variety of treatment procedures. • Taking and developing radiographs (x-rays). • Asking about the patient’s medical history and taking blood pressure and pulse. • Serving as an infection control officer, developing infection control protocol and preparing and sterilizing instruments and equipment. • Helping patients feel comfortable before, during, and after dental treatment. • Providing patients with instructions for oral care following surgery or other dental treatment procedures, such as the placement of a restoration (filling).
• Teaching patients appropriate oral hygiene strategies to maintain oral health.
• Performing office management tasks that often require the use of a personal computer.

This issue features Jennifer H from Southeastern Wisconsin. Jennifer has been working as a Dental Assistant for the past three years and is currently employed at two different offices! Q: Why did you become a DA? A: I enjoy working with people and I knew I wanted to do something within the health care field. I coupled that thought with the love I have for teeth and discovered the field of Dental Assisting. I get satisfaction out of making people happy and putting a healthy smile on their face. Q: For someone entering the field or already in it, what qualities do you think make a good DA? A: One of the most important qualities I believe is to have compassion towards people. People from all over will come into your office and while some have had a great day, others may have had a bad day, and you need to be able to adapt and have compassion for everyone that walks through the doors. Another big one would be critical thinking in cases of having to help think outside of the box and provide new ideas to your dentists. The ability to anticipate the doctor’s needs and figure out how to fulfill them is important as well. This will show them that you can think on your feet and that you are an important aspect to the team. A few other top qualities I think will help someone succeed is the ability to multitask and the ability to be organized and help your doctor stay organized. Q: Best aspect of your career? A: I would have to say the patients. Being able to change someones smile or improve their overall oral health makes me the happiest. I have even had some patients cry in the chair because they are so happy with their treatment and how it turned out. Seeing how I can directly impact someone’s life in such a meaningful way makes me love the career I am in. Q: Alright, well there must be something you don’t particularly like or would like to see changed? A: Within the career itself, I would say there really isn’t much I dislike. Office-wise, there are a few petty interactions between co- workers but that is to be expected at any job. But overall, I love the extra responsibilities I am given by my doctor and am happy I work with someone who allows me to go beyond what some doctors may allow their DAs. Q: Sometimes we hear that DAs feel undervalued and not respected by either their doctors or hygienists, do you feel that way? A: No, just the opposite actually. I feel very valued and respected at both of the offices I work at. Both doctors will tell patients how much they appreciate me as their DA and as a person. I do not
think that any one at either of my offices looks down on me or my skills. I believe that everyone I work with sees the hard work and dedication I put into my career and I am rewarded with their respect. I can see how if you do the bare minimum and do not provide value to the office that either the doctor and / or the hygienists may lack respect for you. Q: How involved within the office are you?What responsibilities do you have? A: I am responsible for buying products and then restocking the office, scheduling appointments, working with the insurance companies, organizing and cleaning the office and patient care. I work alongside the doctor and help design ceric crowns and temporary crowns. I also chart for the hygienist and help them with instruments. As mentioned before, my doctors give me a lot of responsibilities that some DAs may not get. Q: Salary wise, do you believe you are paid a below average, fair/average or above average salary compared to the work you are asked to perform? A: For myself, I believe I am paid a fair wage based on everything I am responsible for and how much value I believe I bring to my doctors’ practice. On the ends of the spectrum I have heard other DAs saying that they make about $13 an hour to others who are making $23 an hour, those being the lowest and highest I have heard. If you believe you are making less than you think you deserve, make sure you talk to your boss and lay out specific examples of why you should be earning more based on your performance. If you are earning towards the lower end of the salary range, ask yourself what else you can do in order to make more, i.e. ask for more responsibility or become more efficient. I think there are always ways to improve your salary within this career field. Q: What skills have you acquired or strengthened since becoming a DA three years ago? A: I think I have picked up a better sense of understanding. As mentioned before, people of all different backgrounds come into the office and you have to learn to be patient and understanding to everyone as to not offend someone. I also have a better appreciation for the need of continuing education. The health care field is constantly innovating and in order to keep up I am constantly ready magazines, new paper and online articles, something I never imagined I would be doing in a career choice after I graduated from school. Q: Any advice for people just starting their career or questioning if they made the right choice? A: Do not pick this career if you are not all in. By that I mean do not just do it for a paycheck, you will probably be disappointed. Make sure you are doing this because you love helping people and making others better. Dental Assistants have the opportunity to touch so many people’s lives in a positive way as long as they enjoy what they are doing. People who have chosen this career for money or for an “easy” career are going to be in for a shock.

YOUR ROLL IN INFECTION CONTROL
Dental assistants are responsible for some of the most important tasks in a dental office. They keep the office running smoothly, sterilize instruments, setup and manage appointments, getting rooms ready for patients and the list goes on and on. As ones who keep the office humming along and love their work with helping patients, Dental Assistants might not be aware nor fully cognizant of the risks of exposure incidence. Incidence and the associated risks are serious and pose serious health issues with potential infection of HIV, hepatitis B, C and D along with other diseases. This article focuses on the association implications and insights on what can be done to manage and minimize risk of infection. Infection control is a key responsibility for Dental Assistants in that according to DANB’s latest Dental Assistant Survey, 81% of dental assistants who hold DANB’s Certified Dental Assistant (CDA) certification are responsible for infection control-related tasks making it first in the list of Top 5 Duties CDA Certificants Perform (http://danbcertified.org/): 1. Infection control – 81% 2. Lab functions – 70% 3. Order dental supplies – 63% 4. Expanded functions – 58% 5. In-office training – 41% The Dental Assisting National Board (DANB) and its affiliate, the DALE Foundation, are dedicated to public protection and know that dental assistants play an important role in patient safety, in addition to their own safety. While there is a chance for an infection, prior research shows that the risk of infection is low. One study published by the American Dental Association shows this is as low as 0.3%. While this is relatively low, it is not zero. “It`s more than just HIV,” says Kathy Eklund, RDH, MHP, associate professor at Forsythe School for Dental Hygiene. “There`s hepatitis B,
C, D, and many other diseases. You`re still at risk. There is still that 0.3 percent chance of HIV.” There are actually three modes of transmission that are of concern which are the following: 1. From the healthcare providers to the patient. 2. From patient to patient. 3. From the environment, especially from water sources, to the patient.
Dental patients and dental healthcare providers can be exposed to bacteria and viruses, dental patients and dental healthcare providers can be exposed to bacteria and viruses, including influenza, HIV, hepatitis B and C, cytomegalovirus (CMV), herpes simplex, tuberculosis, staphylococci, and streptococci, as well as a myriad of other
bacteria and viruses that inhabit the mouth. These bacteria and viruses can be transmitted to the patient through direct contact with blood or oral fluids, indirect contact with contaminated objects, or inhalation of microorganisms that can remain suspended in the air for prolonged periods of time. The careful use of standard precautions is instrumental in preventing the transfer of these microorganisms. OSHA is a regulatory agency that ensures workplace safety with enforcement capability, allowing its representative to investigate and impose fines. Two OSHA standards that dental offices must comply with are: the OSHA Bloodborne Pathogen Standard (29 CFR 1910.1030) and the Hazard Communication Standard (29 CFR 1910.1200). Table 1 below describes the required elements of each standard:

According to the OSHA, Bloodborne Pathogens Standard, an exposure incident is: • A cut or puncture with a contaminated item • Puncture of the skin with a contaminated needle or instrument • Splashing of blood and/or saliva onto mucous membranes (eyes, nose or mouth) • Splash or splatter of blood and/or saliva onto non-intact (chapped or abraded) skin
OSHA requires reporting and recording of such incidents. OSHA requirements further stipulate that dental practices must have a written plan for follow-up and possible treatment of employee if an exposure incident occurs and employers are required to keep records of exposures on file. For OSHA violations, it is important to note the employer pays any fine(s), not the employee. The most common citations issued is related to non-compliance with the Blood borne Pathogen Standard, which involves a starting penalty of $12,675 per violation. Failure to Abate, meaning a practice failed to respond in the timeframe OSHA indicated. The most serious violation is the Willful or repeated violation, which is the result of a practice not having a safety program or having a prior citation that has gone without correction. (Source: Dental Products Report, 6 Scary Consequences of Not Following Good Infection Control Practices, 10/25/2017) Despite the fact that it is a requirement, exposure incidence can go unreported so it is unknown exactly how many incidents in total actually take place. Shockingly, healthcare providers have a myriad of excuses not to report incidences. One of the most common excuses is the lack of education and perceived low risk of disease transmission associated with seemingly minor events. Additionally, certain healthcare providers will make judgements based on the perceived health of their patients and many are afraid that speaking out is a reflection on their competency and fear that it may jeopardize their employment. The Dental Assistant is instrumental with ensuring all persons in the practice adhere to proper procedures and protocol in working with patients, handling of instruments and using consumables, especially those that involve needles or instruments with sharp edges/surfaces. The Dental Assistant is typically the primary person responsible for OSHA infection control products as 87% of the time he/she is the one purchasing those supplies or products (Source: American Dental Assistant Association (ADAA)) that include personal protection equipment such as gloves, masks, protective eyewear and protective clothing. Proper infection control procedures are supported by worker training, which is required annually by OSHA, about the risks of exposure and proper use of personal protection equipment (PPE). Oral health professionals and patients rely on expert infection control guidelines and practices including those for patients from the Centers for Disease
Control (CDC), which provides evidence-based rules, recommendations and guidelines. Their summary of infection control procedures provide recommendations around proper hygiene, precautions, safe injection practices, sterilization and disinfection of devices, and more. This reference document contains an infection prevention checklist that can be used as part of internal audit processes and for assessing the current infection prevention and control procedures for a dental practice. Periodic monitoring via a mock OSHA audit or self-audit done by the clinic team ensures that employees are both doing what they are supposed to be doing and what they’ve been instructed to do. The Dental Assistant serves a vital role for the dental practice in the area of infection control. From an awareness standpoint to procedures and training, diligent activity done periodically will ensure proper procedures are utilized to avoid incidence exposure and protect safety and well-being of both patients and employees involved in practice care.
