Reilly Anderson Marketing & Social Media Coordinator Interim HealthCare of the Twin Cities J ane, age 78, wants nothing more than to have an active lifestyle and be independent in her home. Her goals include staying healthy and not having to rely on her husband or family to assist her. Unfortunately, Jane experienced a progression of symptoms resulting in an infection that led to surgery. Th is, combined with a heart condition, contributed to her weakness, weight loss, and confusion resulting in multiple falls. With limited mobility, she knew she needed help but didn’t know where to go or how to pay for it. Following hospitalization for surgery, she was referred to home care by the hospital discharge planner. Jane’s care started right away when she got home. Due to her homebound status, treatment was covered by Medicare, which included the physical and occupational therapy that was ordered by her doctor. While she knew a move to an assisted living may be necessary in the future, she was hoping to avoid this a little longer. Her home care team worked with Jane to regain her independence.

Jane and therapists dedicate themselves to recovery Week after week, Jane improved, gaining strength after each therapy session. She worked hard with her physical therapists to get back to the way she was before the falls. After seven sessions, Jane was fi nally back to walking independently without an assistive device in her own home and walking with a walker while outside. Th is is what she had been doing prior to her illness. Th is took dedication from not only Jane, but also from therapists who worked with her each week to make

understanding and using words as well as the swallowing function.

sure she could get back on her feet and enjoy her independence. Jane is back in her own environment where she feels comfortable and safe. She is now able to get out and enjoy activities in the community as well as spend time with family at the cabin. Jane also knows that since home care was helpful to her, she will consider it again if she has a need, providing con fi dence that she can live on her own for years to come. Jane is thrilled with her improvement and is once again happily enjoying an independent lifestyle.
