Returns & Warranties

Product Policy Crest offers high-quality products to support customer demands with both manufactured and distributed products. We work with suppliers who pro- vide us with quality components and finished products. Products are often tested to ETL and UL standards; these certifications are noted near individual products within this catalog. When a product doesn’t function to your standards, we want to hear from you and make it right! We can check to see if your product is within its warranty timeframe and repair or replace your item. If it is outside the warranty timeframe, we can help you find a replacement or repair your item in our Crest Repairs Department – see more information on pages 105-106. Return Policy Customers who order a wrong item or are dissatisfied with their purchase may return products within 90 days of the ship date. Customers will receive credit or, upon their request, will be issued a refund. Returns after 90 days are generally not accepted. In order to return a product, the customer must complete the Returns Order Request form found on the Returns page at CrestHealthcare.com. Crest will provide a Sales Return (SR) number. Returns may not be accepted without an SR. Returns after 30 days from the ship date will be subject to a restocking fee of 20%. All returned items must be in new, resellable condition and in the original packaging. Any items returned after 30 days that are not sellable will be returned to the customer at the customer’s expense. Custom made-to-order items, fabricated orders and certain large volume orders may not be returned unless defective. Personal use items are not returnable if opened/removed from the original packaging. Non-stock dropped shipped items with an asterisk ( * ) or are shipped directly from our vendors. Drop shipped orders are subject to the vendor’s return and restocking policy. Purchaser is responsible for all freight charges unless the item was shipped in error or defective. Warranty Icon Icons included with product grouping represent included warranties. A small guide of commonly used icons can be viewed below with additional explanation. Crest warrants that all products manufactured by Crest which do not otherwise have a stated warranty period shall be free from defects in material, workmanship and manufacturing for a period of one (1) year from the date of shipment of the products. All products manufactured by a third party and distributed by Crest are sold to you subject only to the warranties provided by the manufacturer, if any. Any warranty granted in this agreement is for the exclusive benefit of you and shall not accrue to the benefit of any third party. Crest makes no warranties as to the products except as described herein and expressly disclaims all other warranties whether expressed, implied or statutory, including the warranties of merchantability and fitness for a particular purpose, non-infringement or warranties arising from the course of dealing or usage of trade. The warranties set forth above are also subject to the following provisions: 1. The warranties are void and do not cover any products that have been altered, self-repaired, tampered with, abused, misused or used for purposes other than for the use intended by Crest. 2. You are responsible for costs incurred to return products to Crest or a third party manufacturer for warranty claims. Products repaired by Crest under warranty will be returned to you postage paid. 3. A minimum cleaning charge may apply to certain products. 4. Repaired or replaced products will be covered for the balance of the original warranty period, if any. To make a claim under this warranty, simply obtain a Sales Return (SR) number from the Crest Customer Service Team at: 1-800-328-8908 or customerservice@cresthealthcare.com. Return the unit prepaid to Crest Healthcare at 195 Third Street South, Dassel, MN 55325-0727. A Ground call tag will be issued to pick up the products. Please have package ready for pickup. California Proposition 65 Products distributed by Crest Healthcare may contain chemicals considered to cause cancer and birth defects or other reproductive harm under Cali- fornia Proposition 65.
