Repairs Department We provide expert and economical repairs for a broad range of healthcare related equipment. Your products will be cleaned, repaired and tested before being returned to you.

It’s Easy to Use the Crest Repair Service: 1) Call our Customer Service Department on our toll-free number and request a repair authorization (RA) number. Note: Failure to obtain an (RA) number may result in a delay of the processing of your repair request. 2) Include a card or sheet of paper with the equipment to be repaired listing the following information: • Name & phone number of contact person at your facility • Return address (street address, not PO box) • RA number received from Crest • Special instructions, if any 3) Select method of shipping. Ground averages 4-5 days in each direction, or choose 1, 2 or 3 day shipping. 4) Write your assigned RA number on outside of shipping box. 5) Ship to Crest Healthcare Supply, 195 Third Street South, Dassel, MN 55325. 6) Call our toll-free number, 1-800-328-8908, if you have any questions.

If OEM components are required for repair, there will be an additional charge above and beyond the normal flat rate repair fee. Non-Crest bed controls requiring the replacement of cases and structural components are not repairable.

Crest is the authorized repair service provider for Legacy Dukane® products.

A $15.00 minimum service charge will apply to any item with a list price of more than $20 sent in for repair.This $15 service charge also applies to estimate requests. The respective Flat Rate will apply in lieu of the service charge on any device found to be repairable. If, due to age, condition, or cost of replacement parts, a device is found to be unrepairable, Crest may, at its discretion, decline to repair the device. Whenever possible, Crest will attempt to recommend a direct substitute and will waive the $15.00 minimum service charge if replacements are purchased.

Note: Crest Healthcare Supply does not issue call tags for repairs. Send products to us by your choice of carrier. Crest may, at its discretion, waive the minimum service charge.
